Although invitations are issued, the Midwest High School Combine is
open to all athletes who feel they have the talent to play college
football. Due to space and time constraints each combine session is limited to
the first 125 qualified athletes who return ALL of the required
paperwork. This
includes obtaining your transcripts from you high school office or
guidance counselor and returning those with your registration forms.
Please double check that you have filled out all six (6) required forms and have included your registration fee and transcripts before sealing your envelope for mailing. Forms can be downloaded in PDF format by clicking here. You will need Adobe Acrobat Reader to view and print the forms in PDF format. Most computers come with this software and it is also available for free by clicking here
